The Australian Council for Safety and Quality in Health Care  has worked with the Royal Australasian College of Surgeons (RACS) and the States and Territories to develop a protocol for the prevention of procedures performed on the wrong patient or part of the body.

The SA Policy for Ensuring Correct Patient, Correct Site, Correct Procedure Protocol is available on the right hand side of this page.   This Protocol has been adapted with kind permission from the Department of Veterans Affairs National Center for Patient Safety (USA) Directive on Ensuring Correct Surgery.
